Request for Information: CBO and Health Care Partnership Best Practices

The Center for Health Care Strategies (CHCS), the Nonprofit Finance Fund and the Alliance for Strong Families and Communities have released a request for information survey to identify and disseminate best practices of partnership models between community-based organizations (CBOs) and health care systems, especially those serving low-income and vulnerable populations. 


The information gleaned from the survey is intended to advance knowledge about these partnerships as it is increasingly clear that CBOs play an important role in addressing social determinants of health and helping patients access critical services and supports.

The survey is designed to:
1.      Identify promising models and challenges for building effective partnerships;
2.      Inform funding and policies; and
3.      Advance effective partnerships across the nation.

Key findings from this survey will be synthesized and shared widely through publications, webinars and other vehicles. Using survey responses, up to five partnerships will be selected to be showcased nationally as case studies.

The RFI is being funded by the Robert Wood Johnson Foundation and closes Friday, January 27.
