Nominate an Innovator for The John A. Hartford Foundation Business Innovation Award

A new blog from n4a's Aging and Disability Business Institute encourages readers to submit nominations for the 2017 The John A. Hartford Foundation Business Innovation Award, which recognizes successful partnerships between community-based organizations (CBOs) and the health care sector.

Written by Rani Snyder, MPA, Program Director at The John A. Hartford Foundation, the post highlights the significance of CBO-health care collaborations in improving the quality of life for older adults and people with disabilities.  
 
As The John A. Hartford Foundation works to build age-friendly health systems, its collaboration with n4a and other Business Institute partners on the Business Innovation Award will identify partnership models that deliver integrated, coordinated care across all settings, including the home and community.
