An Opportunity to Shape Care Planning to Include LTSS
This is important and needs your input on improving care planning for people with complex medical needs or disabilities. The electronic Long-Term Services & Supports (eLTSS) Initiative is a partnership between the Office of the National Coordinator for Health Information Technology (ONC) and the Centers for Medicare and Medicaid Services (CMS). This project is working to identify and harmonize electronic data standards that can enable the creation, exchange and re-use of interoperable service plans by health care and community-based LTSS providers, payers and the individuals they serve.
Through August 2017, the eLTSS project will hold public weekly meetings on Thursdays at 12:30 - 2:00 pm ET. These meetings are open to the public and present an important opportunity to provide input into the development of data standards for eLTSS care plans. You can easily register as an "interested party" by sharing your email address.

The goal of the project is to develop a national data standard for LTSS care plans that will allow these services to be combined in a modular fashion with other health care data in comprehensive care plans for coordination, planning and use of health and psychosocial services by individuals who receive care from multiple providers on an ongoing basis.
