Nominations for The John A. Hartford Foundation 2019 Business Innovation Award Open!

The Aging and Disability Business Institute is excited to announce that it is accepting applications for the 2019 John A. Hartford Foundation Business Innovation Award.

Now in its fourth year, the John A. Hartford Foundation  Business Innovation Award usiness Innovation Award celebrates innovative aging and disability community-based organizations (CBOs) that are making transformative strides toward a system of integrated care by partnering with health care entities. When health care entities and CBOs work together in a meaningful and sustainable way to support health, the quality of care improves, cost savings accumulate, and older adults and people with disabilities are better able to live in their homes and communities.

Past winners of the John A. Hartford Foundation Business Innovation Award include the Southern Maine Agency on Aging (SMAA), the Virginia Area Agencies on Aging–Caring for the Commonwealth (VAAACares) and the Coalition of Limited English Speaking Elderly (CLESE).

Growing evidence shows that most health care costs  are driven by factors outside of the clinical environment, including the social determinants of health—such as food security, housing instability and quality, and utility and transportation access. By addressing these issues through partnerships, CBOs and health care entities can optimize efficient and effective integrated care for older adults and individuals who have disabilities.

The 2019 The John A. Hartford Foundation  Business Innovation Award will be presented at n4a’s 44th Annual Conference in New Orleans July 27-31, 2019 and will include a plaque and monetary award of up to $5,000. Nominees will be notified of the results prior to the Conference.
