The John A. Hartford Foundation Business Innovation Award 2017
Nominations are now being accepted for this year’s Business Innovation Award.
The John A. Hartford Foundation (JAHF) Business Innovation Award was established in 2016 to highlight the successes of community-based organizations (CBOs) that have partnered with health care entities. It is more important than ever to identify innovative and effective programs that help people receive medical, social and behavioral services in a coordinated way.
The award recognizes CBOs that have successfully partnered with health care sector entities, and have a track record of developing bold, transformative initiatives designed to integrate health care and long-term services and supports. Qualified submissions will demonstrate successfully implemented and sustainable partnership models that have resulted in improved quality of life for older adults and/or people with disabilities.
In July 2016, the inaugural The JAHF Business Innovation Award was presented to the Southern Maine Agency on Aging (SMAA). Honorable mentions went to Aging & In-Home Services of Northeast Indiana (AIHS) and Aging and Independence Services of the County of San Diego (AIS). SMAA, AIHS and AIS are trailblazers in the field of integrated care, effectively bridging the divide between social services and traditional medical care.
