n4a’s Aging and Disability Business Institute will give an overview of how the emerging cross-sector focus on social determinants of health will impact the relationships between community-based organizations (CBOs) and various health care entities.
The growing attention being paid to social determinants of health by payers, providers, and policymakers is creating new incentives that will encourage and catalyze partnerships between CBOs and traditional health care organizations. However, these relationships are still in the early days of exploration and implementation, and there are major issues of culture, infrastructure, operations, and payment that will need to be bridged in order for meaningful integration to come about.
Brent T. Feorene, MBA, Health Care Consultant, will lead this one-hour webinar highlighting the importance of the social determinants of health and discussing how health care can collaborate with CBOs to ensure consumer access to this vital segment of the care continuum.
